If bring-in bet is part of the game, then he who has the lowest up card pays it and betting starts. Sometimes, the Fourth Street betting round is scrapped by dealing 2 hole and 2 up cards in the opening round, which makes Fifth Street the next betting round after an up card has been dealt.
